How can I find out client IP address,its username,website visited and time and date

Use Server Variables.  

A list of the available ones is in the link above also included is 
usage and information on each.

I use a stored procedure to log all users, ips and the like.  There are 
many many ways to do this.  As for the username of the client if you've 
given the a unique username and it is stored in a session you will have 
to call it as a Session Variable.
