The patch program looks at the patch file and makes sure that it matches the file being patched. For example, a patch file might contain 2 changes where the first one changes a CVS revision ID line, and the second one deletes a line containing "free(field_name);". The lines that change are preceeded by a + or - to indicate if the specified line should be inserted or deleted.
