How to differentiate "refreshing a window" and "closing a window" events

Essentially, you cannot. To refresh the window browsers unload it then
reload it from the cache, or you can force a reload from the server.
What this implies is that to refresh a page, it needs to be unloaded
first before it can be reloaded. It is this that causes the most
confusion, to reload a page must be unloaded. 
Sorry, this is the best answer around at the moment, but you never know
what the future may hold.
